Student Assignment Packet
This packet is used as an evaluation tool during your internship or cooperative education placement. Packets can be picked up from our office, or you may download the information from the following links. Some of the forms are offered in fill-in versions, which will allow you to type information directly into them before you print them. You can also print the non-fill-in versions, and use a typewriter to complete them. Forms that require a signature MUST be printed and mailed or faxed. All reports must be submitted by the due dates below. Failure to submit all reports constitutes an automatic "U" grade.
