This packet is used as an evaluation tool during your internship or co-operative education placement. Please complete the online forms and submit electronically. If you encounter problems with the electronic submission process, please notify our office at ceoassistant@admin.fsu.edu. If technical difficulties persist longer than anticipated, note that printable copies of the forms are available and can be faxed to 850-644-3273. All reports must be submitted by the due dates below. Failure to submit all reports constitutes an automatic "U" grade.
